New Tech Ally Ready to Fight Against Chronic Health Conditions

Chronic health conditions are the leading cause of illness, disability and death in the U.S. and also impact health care costs. Remote patient monitoring offers one answer to what ails us…and Sutter Health and Epic now offer a way to supercharge those efforts.

Sutter Health will deploy first-of-their-kind medical devices, with a direct data link to Epic’s EHR, to help patients proactively manage their chronic health conditions from anywhere.

Effortless, secure uploads of key clinical data will now help providers make better-informed treatment decisions or suggest timely medication adjustments remotely, with the intent to help those with chronic conditions stay on track with their care plans. As part of the Sutter Sync digital care program, the new Sutter devices will support patients managing high blood pressure, with a pregnancy pilot also launching for a small cohort of patients and referring providers. At the same time, the program is expanding this fall to support eligible Medicare patients with high cholesterol, with Type 2 diabetes support planned for early 2026. Eligible patients are referred to the program through their physicians.

Focusing on chronic conditions – which oftentimes lead to other serious health threats like heart disease or stroke – couldn’t come at a more crucial time. About 55% of California adults have either prediabetes or diagnosed diabetes, according to the American Diabetes Association. Nearly 37% of adults in California have high cholesterol.
